City of Gardendale Permit for food trucks and push carts
All food trucks and push carts (basically anything on wheels) must complete the form below and send us all the required documents listed on this form. For questions about the required information, please call Jay Brown at city hall 205-631-8789.
Food trucks and push carts will now be required to have a City of Gardendale business license. If you do not need the business license for the full year, the one day special event fee on the business license is just $34.
Fire safety requirements
Use this inspection form to ensure you are abiding by the fire and safety requirements.
Menu List
Fill out the required menu list. Do NOT attach a prepared menu list. This list goes to the Health Dept on this form only. 2026 document available January 6.
